Australia’s Expedited Specialist Pathway Expands in 2026: General Medicine & General Paediatrics Added

The Expedited Specialist Pathway (ESP) in Australia has officially expanded to include General Medicine and General Paediatrics, effective 19 January 2026.


According to the latest announcement from Ahpra and the Medical Board of Australia, well-credentialed, internationally trained physicians and paediatricians from comparable health systems can now access this fast-track specialist registration pathway.


This is one of the most significant updates to medical registration in Australia in recent years, particularly for UK and Irish-trained specialists, international medical graduates (IMGs), and doctors seeking specialist jobs or locum work in Australia.


Why These Specialties Were Added


The new specialties were nominated by Australian health departments based on national workforce priorities, with an urgent need for experienced specialists across public hospitals, regional services, and acute care settings.


🆕 New Priority Specialties


The newly approved ESP categories include:

  • General MedicineTargeting specialists with General Internal Medicine or dual training in Acute Internal Medicine, primarily from the United Kingdom
    • General PaediatricsFocused on experienced paediatricians from similar health systems, including the UK and Ireland


This expansion reflects growing demand for general physicians and paediatric specialists across Australia’s healthcare system.


Existing & Upcoming Specialties on the ESP


The Expedited Specialist Pathway now includes the following specialties:


  • General Practice – added October 2024
  • Anaesthetics – added December 2024
  • Psychiatry – added December 2024
  • Obstetrics & Gynaecology – added March 2025
  • General Medicine – added January 2026
  • General Paediatrics – added January 2026


🔎 What’s next?

Qualifications in Diagnostic Radiology are currently being assessed by the Australian Medical Council (AMC) and are widely expected to be added next.



A Quick Reality Check on the Expedited Specialist Pathway


The ESP has generated debate since its introduction, with some medical colleges publicly criticising its rollout and its independence from traditional college-led specialist recognition.


Despite this, the pathway is:

  • Fully endorsed by AHPRA and the Medical Board of Australia
  • Already enabling specialists to enter the Australian workforce faster
  • Actively being used to address critical workforce shortages

  • Faster access to specialist registration in Australia
  • Lower cost than traditional college pathways
  • Reduced administrative burden
  • Earlier access to specialist employment and locum doctor jobs


⚠ Important Consideration


  • The ESP does not automatically confer Fellowship (e.g. FRACP)
  • Doctors seeking Fellowship will usually still need a separate RACP assessment after commencing work
  • Fellowship remains the pathway to full equivalence, broader private practice rights, and unrestricted locum work (with caveats



Qualification & Training Requirements


🩺 General Medicine – UK Trained Specialists Only


At present, the General Medicine ESP is limited to UK-trained specialists who have completed CCT (not CESR).


Core Requirements

  • MRCP (UK)
  • CCT in General Internal Medicine

Eligible Training Pathways

  • August 2007 – Present Stand-alone General Internal Medicine (JRCPTB GIM / IM Stage 1 & 2)
  • August 2009 – Present Dual Training in General Internal Medicine + Acute Internal Medicine
  • August 2019 – Present General Internal Medicine within an approved Dual CCT pairing

📌 Important:

This pathway applies to General Medicine only. Doctors intending to practise in a medical sub-specialty (e.g. Cardiology, Geriatrics, Respiratory Medicine) must apply through the traditional RACP Specialist Recognition pathway for that sub-specialty.



🩺 General Paediatrics – UK & Ireland Trained Specialists


The General Paediatrics ESP is open to both UK and Irish-trained specialists.


Ireland – Accepted Qualifications

  • MRCPI (Paediatrics) – July 2010 onwards
  • CSCST in Paediatrics
  • Completion of Specialist Training (SAT) under the RCPI Faculty of Paediatrics


United Kingdom – Accepted Qualifications

  • MRCPCH – August 2007 onwards
  • CCT in Paediatrics
  • (issued by PMETB pre-2010 or GMC post-2010)
  • Completion of any recognised RCPCH curriculum


Additional Mandatory Requirements


All General Paediatrics applicants must demonstrate:

  • Recent and relevant general paediatrics clinical experience
  • Advanced Paediatric Life Support (APLS) or equivalent
  • Completion of an Australian Child Protection Course



Core Registration Requirements Still Apply


Specialists approved through the Expedited Specialist Pathway must still meet standard Australian registration requirements, including:


  • Six months of supervised practice
  • Completion of an orientation to the Australian health system
  • Mandatory cultural safety education


These requirements ensure patient safety while supporting a smoother transition into Australian clinical practice.
